to be able to Submit each Application.

How to Apply
Applying is easy using our Online Application. We require families to apply online as it simplifies the submission process. A non-refundable fee of $50 is submitted with each application.
Create an Online Application
To begin the Online Application process,
Create an Account. Next, log in to your account and
Create A New Student Application for your child. You will then have the flexibility to log in and out of your account and access your open application.
Be Prepared! You will be required to upload each Applicant's "most current" State of Florida Immunization Form DH680 to be able to Submit each Application.
